While we realize that the pandemic is not over, we are prepared to rebuild student learning experiences and welcome our parents back onto campuses. Please note, the School will be requiring a Back to School Declaration Form to be submitted for each student prior to returning for any on-campus back to school activities.

Back to School Declaration Form
Prior to arriving on campus for your student’s/students’ first day back to school or any back to school activities, please check for symptoms and complete the short screening form linked below (one per student). Be sure to click “Submit” at the end of the form at the bottom of the last page. Please note, this screening form is only for your student’s/students’ first day back on campus; this will not be a daily requirement.
